OUR POLICIES

Thank you for choosing Memorial Heights Family Medicine for your primary care needs. We are committed to providing you with quality and efficient health care. Below are our office and financial policies. A copy will be provided to you upon request.

OFFICE HOURS:

 

Our office is open Monday through Friday from 7:30 a.m. to 3:00 p.m. 
Patient appointment hours are available from 7:30 a.m. to 1:00 p.m.
Office staff is available from 7:30 a.m. to 3:00 p.m.

EMERGENCIES:

 

If an urgent medical problem arises during a time when the office is closed, call 800-507-2809, and the answering service will contact the doctor on call.  Please note that prescription refills and referrals will be addressed the following business day.

SCHEDULING APPOINTMENTS:

 

Appointments can be scheduled during regular business hours by contacting the office at 713-461-4101. Appointment reminders will be texted to your mobile phone number the day before your appointment.

CANCELLING AN APPOINTMENT:

 

If you cannot keep your appointment, please give us at least a 24-hour notice or you will be charged a $25 no-show fee.  Additionally, if you will be unavoidably late for your appointment please let us know. If you arrive later than 15 minutes, your appointment will be rescheduled.

TELEPHONE CALLS:

 

When calling with a problem or question, please describe the situation in detail to our staff. This will help the doctor determine the nature of the problem and respond quickly and accurately to your message.  A staff member or doctor will return your call as soon as possible after the provider reviews your chart.  All non-emergency calls will be returned after the last patient appointment of the day.

PRESCRIPTIONS:

 

All prescription refill requests require a 48-hour notice. Prescriptions for new medications will not be prescribed over the phone, as an accurate diagnosis is usually possible only with an office visit.  Controlled substances will not be called in after hours or on weekends.  Dr. Nguyen does not write prescriptions for chronic pain medicines.  Dr. Nguyen does not prescribe medications for ADD or ADHD such as Vyvanse or Adderall. 

TEST RESULTS:

 

Should you have any laboratory work or other diagnostic testing done through our practice, you will be notified of the results as soon as they are available (usually within 5 working days from the test date). All results must first be reviewed by Dr. Nguyen.
 

INSURANCE AND PAYMENT POLICY:

 

Your health insurance contract is between you and your insurance company. Knowing your insurance benefits is your responsibility. Any questions or complaints regarding your coverage should be directed to your insurance carrier.  If you are not insured by a plan we are contracted with, payment in full is expected at the time of service. Co-payments, coinsurance, and deductibles are due at the time of service. Payments can be made by VISA, MasterCard, Discover, American Express, or cash. Please be aware that some or all of the services you receive may be non-covered or not considered necessary by your insurer. Payment for services of this nature will be required at the time of service.
